Documentation
An HICL Requisition or other suitable document must accompany all specimens. Specimens must be labeled with a unique HICL Accession Number or your own unique sample number, the patient's name and any other data necessary for identification (e.g. if stimulation test, also indicate collection time). HICL cannot process specimens that are not properly labeled. Patient's full date of birth (e.g. 01-Jan-2000), sex, 24 h urine volume, collection date and all other information should be supplied on the Requisition as requested to help in the interpretation of results. Place requisitions and other documentation in a separate plastic bag to avoid contamination in case of accidental specimen leakage. Retain the back copy of the HICL Requisition for your records. Refer to the HICL Accession Number or your own specimen number when you make an inquiry.
Alternatively, demographic data and test orders can be transferred directly to our Laboratory System via an HL7 compliant interface. Please contact our Information Systems Coordinator at (416) 422-3000 or (888) 285-7817 Ext. 242 or is@hicl.on.ca for further information.
Chain of custody documentation, confirmation of positive findings and tamperproof container systems, may be recommended and provided for some testing of substance abuse rehabilitation, employment, occupational or environmental programs.
